Understanding the consent forms
First Aid
Our First aid consent form allows Knockout Potential staff to administer basic first aid to a child during sessions if required. This may include cleaning and dressing minor wounds, applying ice packs, and providing immediate care until a parent, guardian or emergency services arrive. In the event of a serious injury or medical emergency, emergency services may be contacted.
It's essential to understand that by signing this form, you are granting authorisation for first aid to be administered.
Photography Consent
A photography consent form is your permission for us to take photos or videos of you (or your child) and use them for specific purposes such as:
